Your patients can simply contact us directly by dialing 514-419-3005 or by emailing us at info@medipsy.ca. 

To refer a patient to Medipsy, you may download this form, complete it, then fax it to 514-507-2009, email it to info@medipsy.ca, or simply give it to your patient.  We are located at 4610 Ste Catherine Street West in Westmount.

If you would like to receive free referral pads, please send us an email at info@medipsy.ca or call us at 514-419-3005. We will send you the referral pads by mail, free of cost.
